Skin Infection: Actinomycosis, Mycetoma
Actinomycosis:- This is also a deep fungal infection cause db Actinomces israeli. There are three forms clinically, depending on the sites affect6ed. They are cervicofacial, thoracic and abdominal tpes. The facial form may follow a tooth extraction or some injuries and manifests as a painless hard swelling. It ma mimic a malignant disease and is often called lump jaw. Common antibiotics like pencillin, erythromycin and tetraccline are found to be effective.
Mycetoma:- It is endemic in some areas. It is caused either by true fungi (eumycetoma) or by actinomycetes (actinomycetoma). The common agents producing eumycetoma are Madurella grisea (black grains), M. mycetomi (Black grains) and Allesheria bodi (White grains). Actinomycetomas are caused b Nocardia asteroides (white grains) streptomces somaliensis (white grains) S.pelletieri (red) and S. madurae (white). Clinicall mycetoma usually develops on the foot as multiple nodules with discharging sinuses. Some fungi are sensitive to chemotherapeutic agents like Pencillin. (A. israeli), sulfonamides (N.asteroides) and dapsone (N.brasiliensis.) In most cases surgical intervention ma be needed.
